How to Add a Diagnosis Code in SimplePractice
(Required for Billing Insurance Claims)
? Purpose:
Adding a diagnosis code is necessary for billing. Claims cannot be submitted without a valid diagnosis on file in the client’s profile.
? Step-by-Step Instructions
1. Log Into Your SimplePractice Account
Visit simplepractice.com
Enter your provider login credentials
2. Navigate to the Client’s Profile
Click on “Clients” in the left-hand menu
Select the client you are working with
3. Click on New > Diagnosis & Treatment Plan
On the "Overview" page of your patient chart, select "New" and then "Diagnosis and Treatment Plan"
A Diagnosis section will generate
4. Enter Diagnosis Code(s)
In the Diagnosis Code field, begin typing:
The ICD-10 code (e.g., F41.1)
Or the diagnosis name (e.g., Generalized Anxiety Disorder)
Choose the appropriate option from the dropdown list
You can enter more than one diagnosis. Drag to rearrange them if needed.
- To add another diagnosis code select the plus sign next to the first diagnosis
5. Set Primary Diagnosis
Make sure the most relevant diagnosis is listed first, as this will be the primary diagnosis submitted on claims
6. Click Save
Once your diagnosis entry is complete, scroll to the bottom and click “Save”
⚠️ Important Reminder:
A diagnosis code must be entered and saved in the client’s profile in order for us to submit insurance claims on your behalf. Claims cannot be billed if a diagnosis code isn’t indicated for the client.
Tip for Providers:
You can also add or update a diagnosis while in the timeline of your patients overview page. Hover over the existing "Diagnosis and Treatment Plan" and just click “Edit Diagnosis” within the note, and it will take you directly to the treatment plan section where you can make changes
