What is CAQH?

CAQH is an online data repository of credentialing data. Practitioners self report demographic, education and training, work history, malpractice history, and other relevant credentialing information for insurance companies to access.

How do I get Started?

How to get a CAQH Number (reviewed in detail below):

  1. Register with CAQH ProView.
  2. Complete the online application and review the data.
  3. Authorize access to the information.
  4. Verify the data and/or attest to it.
  5. Upload and submit supporting documents.


Here are the detailed steps to expect when you access the site:

1. To Register, please access: https://proview.caqh.org/PR/Registration

This is the information they will be asking for from you:

1. Basic Personal Information
2. Education and Training (as applicable, WILL vary by license)

  • Medical/Professional School
  • Graduate School
  • Internships and Residencies 
  • Fellowships and preceptorships
  • Teaching Appointments

2. Specialties and Board Certifications (as applicable)
3. Practice Location Information (again, these are all as applicable.  If you do not have a partner, there is no need to list one)

  • Practice Name and Type
  • Address and contact information
  • Billing, office manager contacts (you can use Help Therapy)
  • Services, certifications, imitations and hours of operation 
  • Partners and covering colleagues

4. Malpractice Insurance Information
5. Work History and References (as applicable)
6. Disclosure and Malpractice History (as applicable)


(A lot of this may not apply to you, i.e. hospital affiliation, etc.)

Here is the material you should have ready going into the application process:

1. IRS Forms (W-9)
2. Drug Enforcement Administration Certificate (DEA) - If applicable
3. Controlled Dangerous Substances Certificate - If applicable
4. State medical license(s)
5. Various identification numbers (UPIN, Medicare, Medicaid) - if applicable
6. Malpractice Insurance Policies

Again, if it doesn't apply to you as a provider, you will not need it.

These are the general steps you can anticipate taking (further broken down below):

1. Register with the system
2. Complete All Application Questions
3. Complete Any outstanding required fields
4. Review your application data summary
5. Authorize participating orgs to access your application data (Make sure you indicate BSC - Blue Shield - to process for CCN)
6. Attest to your Application data
7. Upload any missing supporting documentation

This should not take the two hours that it states on the application if you come with all of your documentation ready. Once you are in the system, you will be able to easily update information.

Here is a more thorough breakdown of the steps you can anticipate taking:

1. Go online to https://proview.caqh.org/PR/Registration
2. Click 'Register Now' or 'Go to Next Section' to begin the process
3. Enter your authentication data (SSN, License #, etc.).
*NUCC Grouping is Behavioral Health & Social Services
**If you do not have a DEA number, just select the button (do not have this)
4. Create Username and Password (save this information as this can be used for credentialing with other carriers
5. Choose and answer 3 security questions
6. Click "Acknowledge Terms of Service"
7. Click "Create Account"
8. Select "Profile Data" from the Navigation Bar
9. Enter requested information in each section, use "Go to Previous Section" or "Save and Continue" to go forward and backward on your application. Always "save" so you don't lose information!
10. Select "Review and Attest" to review your profile and make any adjustments you need to. During Review, you can fix any information that needs to be changed.
11. Authorize POs to grant them access to your information (you must authorize BSC (Blue Shield) to give them access to your information and complete Community Care Network (CCN) Registration)
12. Click "Save"
13. Click "Review and Attest"
14. After your attestation, CAQH Proview enables you to upload any required supporting documents directly into the system. You can also upload these as you are completing your application.  So, if you have already done this, please ignore this step.
15. Click on "Documents", "Review" to see if you have any missing documents
16.  You will be notified by Triwest once your contract is cross-signed and approved through the CCN Network using the CAQH data.
