Request for Services (otherwise known as RFS form) are required to continue working with your veteran after the expiration of your current authorization.
When submitting the RFS, please include a second page that includes a summary explaining the need for services making sure to indicate if you are requesting continuation of care if applicable, in addition to the last round of medical documentation/progress notes taken.
For the summary, I recommend structuring your request in the following format: Situation, Action, Result
Situation: What symptoms is your Veteran currently presenting that requires you to request additional services Action: What treatment plan would you like to have done under this request: Medication management, sleep study, advanced testing, etc.
Result: What are you hoping to accomplish with this new course of care/how do you believe this will impact the existing authorization you have.
Example: As the Veteran suffers from the addition of medication will allow in his therapy treatment.

Please NOTE: filling out and sending in this form does not guarantee that you will be granted additional sessions with your clients. You should wait to continue services with your clients until you are approved for a new authorization or risk not getting paid for your services.
