Blue Shield Provider Services is now handling the updates of all information pertaining to your provider profile. This information is then given to Triwest Provider Services.
To update the address, you will fill out the attached Individual Provider Change Form.
Please fill out the form and email it to BSCProviderInfo@blueshieldca.com and cc providerrelations@helptherapy.com so we can update your profile with Help Therapy.
If you are changing anything having to do with your TIN or EIN number (including billing address), you should also submit a signed fw9 with the updated billing address.
